Awareness and usage of ECPR are increasing all over the world. The 2019 focused update on ACLS guidelines 1 addressed the use of ECPR for cardiac arrest and noted that there is insufficient evidence to recommend the routine use of ECPR in cardiac arrest. However, ECPR may be considered if there is a potentially reversible cause of an arrest that would benefit from temporary cardiorespiratory support. The use of ECMO in arrested patients has been referred to as extracorporeal cardiopulmonary resuscitation (ECPR). Abstract PURPOSE OF REVIEW: Extracorporeal cardiopulmonary resuscitation (ECPR) is a contemporary resuscitation approach that employs veno-arterial extracorporeal membrane oxygenation (VA-ECMO).

CONCLUSIONS: In selected patients with refractory cardiac arrest, ECPR may provide temporary support as a bridge to intervention or recovery. We report favourable survival and neurological outcomes in one third of patients and pre-ECMO lactate levels predictive of mortality.
